Community Health & Well-Being Survey

Welcome to the New Canaan Community Health & Well-being Survey

The New Canaan Behavioral Health Alliance (NCBHA) – a coalition of over 30 local organizations dedicated to increasing community awareness of behavioral health resources– is administering this 5-minute Community Health & Well-being Survey from October 1st to 31st. Your Anonymous responses will guide decisions and resources impacting our community’s health, mental health, and well-being. Widespread participation may also unlock federal and state funding for local behavioral health services.

WHY YOUR PARTICIPATION MATTERS

By taking just 5 minutes, you will:

  • Help us understand and address the needs of our community
  • Ensure your voice is heard in shaping local services and programs
  • Help guide decisions that impact the health and well-being of New Canaan residents
  • Potentially unlock additional funding for our community’s needs

Our goal is 100% participation from residents over 18. Make sure every eligible individual in your household completes the survey.
